For example, on E2, when the database went screwy today, giving gods negative XP and the level 1 users Editor status, as well as votes screwing up, not to mention EDB getting taken apart for scrap metal by the chatterbox users.
Happens every so often on big web sites, though I hope this isn't a testament to how good Perl is...
