in the context of BDSM, a spreader (also called spreader bar) is a device for holding a submissive's arms or legs in an open position.
Usually composed of a bar (in some cases of adjustable length) with some sort of manacles fixed at the extremities.

Spreaders can be made at home quite easily, using a broomstick and eyebolts.
Do not confuse spreaders with stocks.

A spreader is a short, unsharpened, round-edged knife. Its blade is usually oval-shaped, and about 5 cm long. The handle may be shorter than other flatware handles. As its name suggests, a spreader is used for spreading jam, butter or cream cheese on bread or crackers. Also called a butter knife, this is probably the smallest utensil you're likely to find on a dinner table.
