In
Gloranthan mythology, the three Chaos gods
Ragnaglar,
Malia, and
Thed. Once, they did horrid things to/with each other (sex is hinted at), and opened the world to
Chaos. That's how Ragnaglar became a
Dead God.
Another version of the Unholy Trio story says that in
Subere's
dark dungeons in Hell, there was
a door that opened on the void of
Chaos, and that the Unholy Trio, thinking there was a
secret power beyond,
forced the door, breaking it forever.
